Summary:
Perform a variety of clerical duties involved in organizing, securing and maintaining participant electronic health records; scanning, attaching, and routing medical records for provider authentication; maintaining associated logs in accordance with established priorities and procedures in any section of the Medical Record Department.
Adhere to HIPAA rules and regulations upholding the strictest of participant confidentiality.
Job Duties:
Sorts, files and collates a variety of medical records and information such as intake documents, enrollment agreements, laboratory and x-ray reports, assessments, physician orders, medical information release forms and all other patient-related documentation into electronic medical records in a timely manner, appropriate sequence and in accordance with established procedures.
Upon request from the clinic, ensures that all filing in the medical record is current and up-to-date for participants upcoming scheduled appointments.
Files and retrieves medical records; researches lost or missing records in accordance with established procedures.
Processes all requests for medical records and reports.
Track and follow-up on outstanding requests and initiates correspondence to all outside medical providers.
Ensures all consults, procedures, and labs are linked to an order in Prime Suite Orders Tracking.
Audit the Returned Status monthly to monitor that orders are being marked Reviewed.
Conducts chart audits to verify accuracy of filing and presence of all required documentation; note any deficiencies and refer to appropriate area for follow-up.
Maintains off-site storage tracking and retrieval.
Answers telephone and takes requests for medical records or information; sorts requests according to predetermined priorities and provides basic participant information as appropriate.
Audits disenrolled/ deceased participant records to ensure completeness including discharge summaries; ensures all notes are signed in Prime Suite and inactivates participant EMR.
Retrieves and copies participant medical information according to HIPAA rules and regulations.
Performs other related duties incidental to the work described herein.
Work requires knowledge of basic clerical and communication principles.
Strong working knowledge of HIPAA/ CMS regulations.
Facilitate the acquisition of medical records from outside vendors.
Performs other related duties as required and assigned.
Qualifications:
High School Diploma or Equivalent, Required Bachelor's Degree, Preferred 1 to 3 years of office support experience, Preferred 1 to 3 years of Medical Records Experience, Preferred 1 to 3 years of Medical Office/Clinical Experience, Preferred 1 to 3 years of EMR Experience (Cognify), Preferred 1 year of experience working with a frail or elderly population, Preferred Certificate in Medical Terminology, Preferred About the PACE Organization of Rhode Island:
